Payment and Insurance

Sam Rodgers is a federally qualified health center and serves all patients regardless of inability to pay. 


We accept most major insurance providers in the region, including Aetna, Blue Cross Blue Shield, Cigna, Humana, United Health Care, MO Healthnet, and MO Healthnet Managed Care plans.  All insurances are subject to the specific plan’s copays, deductibles, or out-of-network increases. Please contact your insurance company prior to your appointment to confirm Sam Rodgers is an in-network provider.

If you have an insurance card, present it during registration, and each time you check-in for your appointment.

Sliding Fee Discount Program

To ensure that care is affordable for all patients, Sam Rodgers offers a sliding fee discount program.  Any patient, whether insured or uninsured, may apply for the sliding fee discount program as outlined by the U.S. Department of Health & Human Services. 

Fees for services will be based on your household income and family size. Charges may be reduced if you live on a limited income according to federal guidelines. We collect annual household income and family size information to determine eligibility for a sliding fee discount. Charges for services that are not covered by insurance may be eligible for a sliding fee discount.

In order to qualify for the sliding fee discount program, patients must complete the sliding fee discount program application and provide the appropriate proof of income. 

Acceptable proofs of income are as follows:

  • Most recent tax return (PREFERRED)
  • Most recent W-2 or 1099
  • Two Most recent check stubs
  • Statement from employer
  • Most recent unemployment check stub or benefits letter
  • Pension, retirement, disability, or social security award letter Temporary Assistance to Needy Families Letter from Division of Family Services
  • Court order settlements (example: child support)
  • Any other written verifiable income statement – self-employment income needs to be verified by accounting records from the business.

Patients will also need to bring the following:

  • Proof of address: This can be a driver’s license (expired accepted), any other form of identification, or a piece of mail (postmarked within the last 30 days).
  • Photo identification: Driver’s license, passport, any government-issued form of identification, etc.

Patients will not receive the sliding fee discount until they have completed their sliding fee paperwork and have been approved by a Sam Rodgers employee. Enrollment must be renewed annually.


We send monthly billing statements. You may pay by cash, check, credit card, or through your insurance. Our billing team is available to discuss your payment options and questions.
