Family is at the heart of everything we do, and so we offer health care for the whole family. We provide other support services that help individuals and families address needs that might fall out of the normal scope of healthcare. If you need help with your taking care of your children, signing up for health insurance, getting transportation to your appointments, access to our pharmacy, or more information regarding our telehealth services, we have options for you.

Pharmacy

Our pharmacy team members collaborate with our providers to dispense safe and effective medications at affordable prices. We serve all Sam Rodgers patients including the uninsured and accept most major insurance plans as well as clinic discounts. We are equipped with experienced, knowledgeable, and friendly staff who are eager to serve our patients.

Population Health

The Sam Rodgers’ Population Health team is an extension of the patient’s provider.  The team consists of LPNs, RNs, Behavioral Health Consultants, Care Coordinators, and Community Health Workers.  All of these professionals work together to ensure that all of our patients receive the highest quality care possible.

Community Health Workers

Community Health workers are professionals who are dedicated to helping patients achieve better health outcomes and live longer, more fulfilling lives by assisting them with their health and wellness goals.  In addition, they are able to connect our patients with other resources which they may need in the KC Metro area.

Outreach & Education

The Sam Rodgers’ Community Access and Engagement team works every day to better connect our patients and community to vital health care services, health education, and other important resources that help break down barriers and promote equitable access.

Health Insurance Services

We want the process of applying for health coverage to be easy for your family! Sam Rodgers provides free application and enrollment assistance for our patients who need help applying for Medicaid/CHIP (MOHealthNet) and the ACA Marketplace.  We also provide this service free to any consumer living in the communities we serve whether they are one of our patients or not.

Telehealth

Sam Rodgers implemented our telehealth services during the COVID-19 pandemic to allow our patients to receive care and connect with our services from their homes. This technology allows us to keep you safe but still connect for the care you need. We will continue to have this technology available to you even after the pandemic since it can be used for a fairly wide of health situations. However, these services are slightly limited and some cases may require an in-person appointment.

WIC (Women, Infants, and Children)

Sam Rodgers has an award-winning WIC team that is dedicated to helping families thrive! WIC is a federally funded program that provides individualized nutrition education and counseling, nutritious foods, breastfeeding education, and other support.
