Dr. Samuel U. Rodgers believed that health care was a basic human right. His vision to ensure quality, accessible care regardless of ability to pay led to him founding the health center fifty years ago.
We're here because we love what we do. Hear from three Sam Rodgers Health Center providers about how they help create a safe space for families.

The Sam Rodgers’ Population Health team is an extension of the patient’s provider. The team consists of LPNs, RNs, Behavioral Health Consultants, Care Coordinators, and Community Health Workers who are dedicated to helping patients achieve better health outcomes and promote longer, higher-quality lives by assisting them with their health and wellness goals.
This program is offered free of charge to existing Sam Rodgers patients.
Some of the things we can assist with are:
Education is the foundation of population health by using technology to develop new models of chronic disease management. We teach patients how to use technology to manage and improve their health, such as automatic blood pressure machines or glucometers (blood sugar testing machines), and record their readings on a log sheet or via app-based trackers.
We have behavioral health consultants who work alongside our medical providers and can assess patients for depression, substance abuse, and other goals to better their health. The primary mission of our behavioral health consultants is to encourage patients along their individual journeys by promoting their strengths which help them to have better overall health outcomes.
Our Community Health Workers and Care Coordinators have access to a variety of resources to assist patients, including:
We provide transportation to and from all service locations. Contact us here.
Our priority is the health of you and your family. You are safe in our hands and your information is confidential.
We respect diverse values, beliefs, and feelings, and can provide care in multiple languages. Our staff, nurses, and doctors share the same diverse backgrounds as our patients and understand your life experience.
We accept Medicare, Medicaid, and private insurance. If you don’t have insurance, we can help you apply or we will determine a cost based on your income and family size. We will never deny health care if a patient is unable to pay.
