What is a Community Health Worker?
Community health workers (CHWs) are trusted members of the community who act as liaisons between patients and the health care system. At Sam Rodgers, our CHWs are here to help patients access medical services, navigate community resources, and provide guidance on managing their health. They play an essential role in supporting patients’ well-being by offering personalized assistance, health education, and a direct connection to local resources. CHWs are especially valuable for individuals who may face barriers to care, such as language differences or limited access to transportation.
What Does a Community Health Worker Do?
CHWs provide personalized assistance to patients, including helping with transportation, coordinating with community agencies, and referring patients to important health programs. Their support extends to navigating facilities, connecting patients to prenatal resources, and ensuring access to nutrition programs such as the Show Me Healthy Women program and WIC.
How Do I Connect with a Community Health Worker?
Community health workers are assigned to the downtown campus, the Cabot Westside campus, the North Oak campus, the Clay County campus, and on-site in the Blue Springs School District.* You can reach a CHW by calling (816) 889.1930 or completing a self-referral here.
*Blue Springs campus CHWs are available only to families with children in the Blue Springs School District.
