We provide other support services that help individuals and families address needs that might fall out of the normal scope of healthcare. Community health workers are professionals who are dedicated to helping patients achieve better health outcomes and live longer, more fulfilling lives by assisting them with their health and wellness goals.  In addition, they are able to connect our patients with other resources that they may need in the KC Metro area. The Sam Rodgers’ community access and engagement team works every day to better connect our patients and community to vital health care services, health education, and other important resources that help break down barriers and promote equitable access. Our pharmacy team collaborates with Sam Rodgers providers to ensure safe and effective medications at affordable prices. We serve all Sam Rodgers patients, including those without insurance, and accept most major insurance plans, along with clinic discounts. The Sam Rodgers’ population health team is an extension of the patient’s provider. The team consists of LPNs, RNs, behavioral health consultants, care coordinators, and community health workers. Sam Rodgers implemented our telehealth services during the COVID-19 pandemic to allow our patients to receive care and connect with our services from their homes. This technology allows us to keep you safe but still connect for the care you need. We will continue to have this technology available to you even after the pandemic since it can be used for a fairly wide of health situations. However, these services are slightly limited and some cases may require an in-person appointment, learn more here. 

Sam Rodgers has an award-winning WIC team that is dedicated to helping families thrive! WIC is a federally funded program that provides individualized nutrition education and counseling, nutritious foods, breastfeeding education, and other support.
