Prepare For Your Visit

We take the guess work out of preparing for your visit.

Prepare for your visit by arriving 30 minutes before your appointment and bring your:

    • Photo ID
    • Insurance card (for any active plans)
    • Medication and vitamin bottles 
    • Immunization records (patients ages 0-19) including COVID vaccination card (all ages, if applicable)

When you arrive, you will check-in with a patient care representative and they will help you with any necessary paperwork and other initial questions. We have live interpreters and other language services available to support you in best communicating your needs throughout your visit.

How To Get The Most From Your Visit:

BEFORE your appointment:

    • Don’t wait until you are very sick or out of medication to call your provider.
    • Have your old medical records sent to us, if it is your first visit.
    • Gather and bring the original bottles for all of your medications and vitamins.
    • When you make an appointment, write down what the scheduler tells you to bring, ask about costs and what to expect at your visit.

AFTER your appointment:

    • If you are not sure about something that was discussed at your visit, make a list of your questions and call your provider.
    • Keep up with your health goals, medication, healthy living and exercise, and always consult your doctor if you have concerns or questions.

Payment and Insurance

When you call to schedule your appointment, you will be asked if you have health insurance coverage.

What if…

I DO NOT have health insurance, Medicaid, or Medicare. Please bring the following documents to your first appointment:

    1. Picture ID
    2. Proof of residency: This document must be no more than 90 days old and have your name, address and date on it. The following documents are approved for proof of residence:
        • A utility or phone bill
        • Mail from a federal, state or local agency.
    3. Proof of income: The following documents are approved for proof of income:
        • A copy of your filed tax return
        • Two (2) recent and consecutive pay stubs
        • Your Social Security earnings statement

I DO have health insurance, Medicaid, or Medicare! Please bring the following to your first appointment:

    1. Picture ID
    2. Insurance Card
