Population Health

Connecting You to Population Health Resources.

The Sam Rodgers’ Population Health team is an extension of the patient’s provider. The team consists of LPNs, RNs, behavioral health consultants, care coordinators, and community health workers who are dedicated to helping patients achieve better health outcomes and promote longer, higher-quality lives by assisting them with their health and wellness goals.

This program is offered free of charge to existing Sam Rodgers patients.

Some of the things we can assist with are:

      • Nutritional counseling to assist with chronic disease and weight loss
      • Asthma/COPD
      • Hypertension (high blood pressure) control and education
      • High lead levels in children
      • Gestational diabetes education
      • Pre-diabetes and diabetes education
      • And more!

Managing Chronic Disease

Education is the foundation of population health by using technology to develop new models of chronic disease management. We teach patients how to use technology to manage and improve their health, such as automatic blood pressure machines or glucometers (blood sugar testing machines), and record their readings on a log sheet or via app-based trackers.

Supporting and Treating the Whole Patient

We have behavioral health consultants who work alongside our medical providers and can assess patients for depression, substance abuse, and other goals to better their health. The primary mission of our behavioral health consultants is to encourage patients along their individual journeys by promoting their strengths which help them to have better overall health outcomes.

Connection to Other Resources

Our community health workers and care coordinators have access to a variety of resources to assist patients, including:

      • Free health technology-based devices such as blood pressure monitors, glucometers (blood sugar testing machines), and more for those that may qualify
      • Health insurance coverage and medical bill payment options
      • Food assistance programs including food stamps, WIC, and local pantry programs
      • Housing and utility assistance
      • And more!
